Campus Facilities Project Request Form

If you're considering a project that involves changes to the college's buildings or grounds, please complete the . This helps the Resource Allocation and Facilities Committee review your proposal to ensure it contributes to a safe, healthy, and welcoming campus environment for everyone.

Course Equivalency

Students may request a and submit to the appropriate school for review. A separate Course Equivalency form needs to be completed for each course unless it is a two part course (Ex: BIO 101 and BIO 101 Lab = Biol 111 so one form is acceptable). Completed forms (forms with both instructor and dean signatures) should be submitted to the Evaluations Office.

Course Registration

If a student needs to register for a course and has a petition or time conflict resolution form, please download and email the form to the student.

Course Substitution

Students may request a through the appropriate school. It is recommended that students work with a faculty member or the Division Dean. Approved forms with all required signatures must be submitted to the Evaluations Office no later than 2 weeks after the end of the term.

Incomplete Grade

This form must be submitted 72 hours after final grades are posted for the semester and an Incomplete Grade was issued to a student(s). A separate form must be completed for each student, for each class where an Incomplete Grade was issued. Completed forms should be submitted to Enrollment Services through inter-office mail, email, or in-person. The instructor is required to contact the student and let them know that they have been given an incomplete, for the course. Forms are held for one year in Enrollment Services and instructors must contact our office if a grade needs to be changed/updated before the expiration date.

Independent Study

allows students to pursue projects under faculty advisement and supervision. To be eligible for Independent Study, a student must be concurrently enrolled in at least one other class at 黑暗爆料APP except for summer sessions. The proposal is subject to prior approval by the supervising instructor and the division dean in order for the student to enroll in Independent Study.

Instructor Grade Correction

Instructor Grade Corrections are instructor initiated and may be used up to one year following the end of a semester to correct a student's grade because of a calculation or clerical error.

Permission to Add After Census

Instructors may submit a Permission to Add After Census to add students (who have attended or participated in class) after the census date has passed. Submit completed forms to Enrollment Services at mjceseforms@mjc.edu

Permission to Drop After Census

Instructors who wish to drop students from full-term or short-term courses after census should submit the Permission to Drop After Census to mjceseforms@mjc.edu.

Positive Attendance Add

may be used to add students into positive attendance courses, after the census date for the course has passed.

Potential Imposter Enrollment

Submit the form if you suspect a potential imposter(s) has enrolled in your course.

Prerequisite/Corequisite Challenge Request

Students may submit to the office of the school in which they are requesting the .

Request for Reinstatement

Request for Reinstatement may be submitted to mjcESeforms@mjc.edu.

Time Conflict Resolution
